In order to provide the best care that we can, it is important for us to know your health history, medications, allergies, and surgical history.

To expedite your visit, we ask that you complete the forms below prior to your visit.  In addition to the attached forms, please bring a referral card (if one was provided to you by your dentist), medication list (if applicable), as well as a list of any allergies (if applicable).

It is important that anyone under the age of 18 must be accompanied by a parent or legal guardian for the consultation visit as well as the surgery appointment.

 

During your initial visit, Dr Munarov will:

            - Review your medical history, medications, allergies

            - Perform a Head and Neck exam as well as an oral cancer screening

            - Explain your diagnosis, treatment options, as well as risks and benefits associated with those                          treatment options

We accept Visa, MasterCard, American Express, Discover, and Debit Cards.

Complete payment is due at time of service unless other arrangements had been made.

We accept most PPO dental insurance plans.  Please call our office to verify if we accept your insurance plan.  We will gladly complete any insurance forms you may have for your reimbursement

We make every possible effort to see our patients on time.  It is common that we schedule more time than we need for someprocedures in order to accomplish this.  Unfortunately, the practice of oral and maxillofacial surgery can be unpredictable, and we appreciate your understanding if we are running late in order to accommodate a  person with an emergency that needs immediate care.

Dr. Munarov is available for emergency oral surgery, even when the office is closed, by calling

(914) 762 6611.  Please inform our answering service that you need to speak to or possibly see the doctor, and he will be contacted.  In the event that Dr. Munarov is away, there will always be a covering doctor. By calling our answering service at (914) 762 6611, you will be directed as to how to reach him.
